Billing
Dr. Kroll is a participating provider in many insurance plans and is accepting new patients. Insurance providers typically cover the costs of office visits and procedures for many medically indicated eyelid and orbit conditions, subject to the terms of a patient’s individual policy (deductibles, co-pays, etc.). Patients are asked to obtain a referral from their primary care provider (if required by your health plan) and are responsible for deductibles, co-payments and remaining balances. Please bring your insurance card with you.
Cosmetic services are not billed through insurance and are payable at the time of the visit. We accept Visa, MasterCard, American Express, Discover, checks and cash.
Cancellations
If you are unable to keep an appointment, we ask that you provide 24 hours of notice. This will enable another patient to use the available time. A fee of $50 will be charged if less than 24 hours notice is given for cancellation or for a “no show”.
